For identity theft using your identity number, please supply the following:

  • Affidavit. Upload your own scanned Affidavit or download and complete the Virgin Mobile Affidavit.
  • Copy of your ID Document. Only a clear presentable picture of yourself

For identity theft using your bank account details, please supply the following:

  • Affidavit. Upload your own scanned Affidavit or download and complete the Virgin Mobile Affidavit.
  • Copy of your ID Document. Only a clear presentable picture of yourself.
  • Recent bank account statement. Upload your most recent one month bank account statement (We accept Internet bank statements).

    We need proof of the fraudulent debit order from your account.
  • What is an Affidavit?
    An Affidavit is any written document in which the signer swears under oath before someone authorised to take oaths, that the statements in the document are true.
